Intensive Insulin Therapy: Has 'tight' control been redefined?

Achieving glucose control by using intensive insulin therapy has become a mainstay of therapy for intensive care patients. The publication of the NICE-SUGAR trial from Australia has added to the evidence to support the value of this intervention, but it also raised concern regarding its safe execution. This webcast reviews the Australian study and the impact the findings have had on the US practices. Target glucose levels in various patient populations will be discussed. Accuracy of glucose sampling and variation in applying various paper and computerized algorithms will be reviewed. Strategies for ICU nurses to deal with the increased workload, adjustment for concomitant therapies and react to changing patient conditions will be discussed. Examples of a multidisciplinary team approach to maintain glucose at target levels while avoiding hypoglycemia will be reviewed.
