Use of care bundles helps improve patient outcomes
A care bundle is a structured way of improving the processes of care — a straightforward set of practices that when performed collectively, can significantly improve patient outcomes for surgeries and catheter-related procedures.
Care bundle for central-line insertion
Education
Hand hygiene
Maximal barrier precautions upon insertion
Skin antisepsis
Optimal catheter site selection, with subclavian vein as the preferred site for non-tunneled catheters
Daily review of line necessity with prompt removal of unnecessary lines
Use of coated catheters
Care bundle for post central-line insertion
Disinfect clean skin with an antiseptic during dressing changes
Change dressings at least weekly for adult and adolescent patients
Do not use topical antibiotic ointment or creams on insertion sites (except when using dialysis catheters)
Do not submerge the catheter under water; showers are permitted if the catheter and connected device are protected with an impermeable cover
Replace peripheral venous catheters at least every 72 to 96 hours in adults to prevent phlebitis and infiltration
When adherence to aseptic technique can't be ensured, replace catheters as soon as possible and after no longer than 48 hours
Replace short-term central venous catheters (CVCs) if purulence is observed at insertion site
Replace CVCs if patient is hemodynamically unstable and CRBSI is suspected
Do not use guidewire techniques to replace catheters if CRBSI is suspected
